Career Opportunities:

Home Health Aide

(Amherst, Greenfield, South Hadley, Chicopee, Westfield)

Requirements:
  • Must be a certified nursing assistant, have successfully completed the certified home health aide or personal care homemaker program OR can show proof of having one years’ experience in the field.
  • Be at least 18 years of age
  • Have proper documentation to work in the U.S.
  • Have access to a vehicle and a valid driver’s license
  • Pass a CORI, SORI and Drug test
  • Has a positive attitude and works well as a team
Responsibilities:
  • Provide personal care services for clients in a professional, caring
    and efficient manner and in accordance with state guidelines
    and per the clients care plan created by the Case Manager
  • Document all care appropriately (i.e., communication logs, incident reports, etc.)
  • Review communication log and discuss any pertinent
    information regarding client at shift change
  • Report to Case Manager for client concerns
  • Report to Scheduler’s for shifts/adjustments in a timely manner
  • Inform case manager of any changes in the client’s condition or needs
  • Assist with Housekeeping duties, including client laundry if needed
  • Prepare, serve or prompt for meals
  • Assist with activities throughout the day
  • Maintain a positive, professional, calming environment for client
  • Adhere to all rules and regulations
  • Complete all required in-service hours needed

Driver

(All locations)

Requirements:
  • Must be at least 18 years of age
  • Must have a valid drivers’ license
  • Must have proof of valid car insurance
  • Must use his/her own vehicle for all transportations
  • Have proper documentation to work in the U.S.
  • Pass a CORI, SORI & Drug Test
  • Is positive, friendly and patient with all clients
  • Must be on time
Responsibilities:
  • Report to the schedulers for shifts, times, etc.
  • Report to the Case Manager for any client incidents/concerns
  • Be on time to all shifts
  • Punch in/out of all shifts
  • Assist clients in and out of vehicle, as needed
  • Submit required mileage forms weekly
  • Able to assess capabilities to determine additional client needs
  • Communicates with office regularly
  • Adhere to all rules and regulations
  • Complete any required trainings/meetings
